Public Safety Service Request Forms
Access Control issues
If you encounter any campus access control issues—such as doors not closing properly, malfunctioning card readers, or alarm problems—please email public.safety@ptsem.edu for prompt assistance. By reporting these issues, you help maintain a safe and secure campus environment.
ID replacement
If you or a dependent lose your PTS ID card, use this service request to report it. To proceed, the student or employee must complete the Public Safety and Service Request Form to request a replacement.
PTS Alumni ID Request
PTS Alumni must present an alumni card to access library resources and enter designated areas on campus, including the parking structure. To begin the process, use the link below to request your alumni card. Keep in mind that you’ll need your PTS network login to access the form.
Key Request
The Key Request is used to have a physical key made for a particular room on campus. This request will require the approval of the Directory of Public Safety and the administrator responsible for the area in question
Visiting Scholar ID request
The Visiting Scholar ID request is for departments hosting a visiting scholar to gather the needed information to start the process for issuing them a PTS ID card prior to the visiting scholar arriving
Van Request
Application to use a PTS Van for official PTS usage. Please refer to the Driving Authorization and Safety Policies located on the Business office section of Inside PTS for more information.
Request for Public Safety staffing
Use this request to schedule Public Safety staff for special campus events that require their presence. For example, you might request coverage for library events held after hours, parking and traffic control, or large public gatherings. In these cases, submitting the form ensures officers are scheduled appropriately. If you need further details or guidance, contact the Department of Public Safety directly.
Request for Public Safety Staffing
Grocery Shuttle
The Seminary runs a limited Saturday shuttle service from Brown Hall and CRW to Nassau Park Pavilion or Mercer Mall. This service helps students who need transportation to buy groceries. Each Saturday, the shuttle runs from 9 a.m. to 1 p.m. It starts at the Templeton Staff Lot on the hour, then stops at CRW (Witherspoon Circle) before heading to either Nassau Park Pavilion or Mercer Mall. Return trips begin at 10:30 a.m. from both locations and run every half hour until 1:30 p.m.
