Public Safety Service Request forms

Access Control issues

To report any issues with campus access control issues such as doors not closing, card readers not working or alarm issues please email public.safety@ptsem.edu for assistance.

ID replacement

This service request can be used if you or a dependent have lost a PTS ID card.  This request can be used for both Student and Employees.  This request must be completed by either the student or the employee.

PTS Alumni ID Request

PTS Alumni cards are needed to utilize library resources as well as access various areas on campus such as the parking structure and other areas. Use the link below to request an alumni card (you will need to use your PTS network log in to access the form).

Key Request

This request is to have a physical key made for a particular room on campus.  This request will require the approval of the Directory of Public Safety and the administrator responsible for the area in question

Visiting Scholar ID request

This request is for departments hosting a visiting scholar to gather the needed information to start the process for issuing them a PTS ID card prior to the visiting scholar arriving

Van Request

Application to use a PTS Van for official PTS usage.  Please refer to the Driving Authorization and Safety Policies located on the Business office section of Inside PTS for more information.

Airport Transportation Request

This request is for requesting airport transportation for Visiting scholars and other related visiting guests. It is not for use to transport PTS employees for reimbursable business travel such as conferences, training and other items covered under the Seminary travel policy

Request for Public Safety staffing

This request is used to request Public Safety Staff for a special event being held on campus that would require to have Public Safety staff designated for the event. Some examples are events in the library after operating hours, parking/traffic control, large events open to the public. For additional information and guidance contact the department of Public Safety

Grocery Shuttle

The Seminary is providing a limited shuttle service on Saturday’s from Brown Hall and CRW to Nassau Park Pavilion or Mercer Mall. This service is meant for use by our students who need transportation assistance obtaining groceries. The Saturday grocery shuttle will operate from 9am-1pm every Saturday, leaving from the Templeton Staff Lot on the hour then stopping at CRW(Witherspoon Circle)and dropping off at Nassau Park Pavilion or Mercer Mall. Return trips will begin at 10:30am at Nassau Park Pavilion or Mercer Mall and will continue on the half hour through 1:30pm.
